Please ensure you have a valid GP referral and call reception on (02) 8789 9008. If the office is closed during business hours (Mon-Thurs 8am-4:30pm) please call or text Dr Teresa Lam’s personal assistant  on 0404 634 142. Please either bring your referral on the day of your appointment or fax to (02) 8789 9009 or contact us via the contact form below.

A valid GP referral is required every 12 months to enable you to claim your medicare rebate.

Telehealth appointments via video-conferencing is available. Please be aware that you may require email and access to a computer.
